HOURS AND TELEPHONE

Our office prides itself on seeing all appointments on a timely basis and has the capacity to see patients on an emergency basis as well. Our phones are answered from 8:30 am to 3:30 pm, Monday through Friday. Our phone number is 816.373.3213, press #1 for the operator. On weekends and holidays, the doctors are on call on a 24-hour basis through our emergency number at 913.599.7459.

WHAT TO BRING

If you are a new or existing patient, please make sure you bring the following to your appointment:

• Insurance card(s)
• Photo ID
• Referrals (if required by your insurance company)
• MRI films, x-rays, CT scans or blood work
• A list of medications that you are currently taking
• Any co-payments that are required at the time of your visit (if applicable)
• Children under 18 must be accompanied by a parent

NOTICE OF PRIVACY PRACTICES

Our Notice of Privacy Practices describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Patients will be asked to sign the Notice of Privacy Practices Acknowledgement.

PAYMENT AND INSURANCE

It is our goal to provide quality health care at the lowest possible cost. Midwest Neurosciences, PC will provide insurance billing. We accept assignment on all Medicare claims. We are not participants with Missouri Medicaid. All balances not covered by insurance are the responsibility of the patient. All co-pays are due and payable at the time of the visit. We accept credit cards, cash or check.

If you have any questions regarding any patient billing concerns, please call us at the number below. However, questions regarding your plan's coverage and benefits should be directed to your employer or insurance company.
